Centre for Transplantation (BMT) and Cellular Therapy

Blood and Marrow Transplants (BMT) offer life-saving treatment for blood cancers, genetic diseases, and immune disorders. Discover types, processes and future advances.

Blood and marrow transplantation (BMT) is a highly sophisticated, life saving medical procedure that offers hope to patients with various life-threatening blood disorders, immune system diseases, and certain types of cancer. The primary goal of this treatment is to replace a patient’s diseased or damaged bone marrow with healthy stem cells, which can then regenerate a new, functional blood and immune system.

Blood and Marrow Transplantation (BMT)

Paediatric

    Common indications

  • Leukemias, lymphomas, sickle cell disease, thalassemia, immunodeficiencies.
  • Key considerations

  • Growth impacts, long-term effects, fertility preservation.
  • Donor selection

  • Siblings preferred, cord blood common.
  • Conditioning

  • Often reduced-intensity to minimize long-term effects.
  • Focus

  • Family-centered care, school reintegration.

Adult

    Common indications

  • Leukemias, multiple myeloma, lymphomas, myelodysplastic syndromes.
  • Key considerations

  • Comorbidities, previous treatments, work/family responsibilities.
  • Donor selection

  • Matched unrelated and haploidentical donors more common.
  • Conditioning

  • Myeloablative or reduced-intensity based on patient factors.
  • Focus

  • More severe GVHD, focus on managing late effects and quality of life.

Bone Marrow Function

  • The bone marrow transplant procedure is a painless process that doesn't require anesthesia. Stem cells are infused into the patient's body through a catheter surgically placed in a chest vein. Following the transplant, patients are closely monitored to prevent infections and complications.
  • Doctors provide guidance on medications, diet, and lifestyle changes. The recovery process involves regular follow-ups for months or even years after the procedure. Frequent blood tests are conducted to track blood counts, which indicate the transplant's effectiveness.

Reasons for Transplantation

    Syngeneic

  • This type of transplant uses stem cells from an identical twin of the patient.
  • Autologous

  • This type of transplant uses the patient's own stem cells, which are collected and stored before high-dose treatments.

    Allogeneic

  • This type of transplant uses stem cells from a donor who is not genetically identical to the patient. The donor can be a related family member or an unrelated individual with matching human leukocyte antigens (HLA).

Types of Transplants

    Autologous:
Using the patient’s own stem cells

  • Collected before high-dose chemotherapy or radiation. Used in multiple myeloma, some lymphomas

    Allogeneic:
    Using donor’s stem cells

  • From a family member, unrelated donor. Used in leukemia, genetic diseases

    Haploidentical:
    Using half-matched family donor

  • Parent, child, or sibling with 50% match. Widens donor pool for those without full matches

Transplant Process

    Pre-transplant

  • Tests to assess health and find a donor.
  • High-dose chemotherapy/radiation to destroy diseased marrow.

    Transplant

  • Infusion of stem cells through IV (like a blood transfusion)
  • Cells travel to bone marrow and start producing new blood cells

    Post-transplant

  • 2-4 weeks for new marrow to produce enough cells
  • Anti-rejection drugs for allogeneic transplants
